I'm going to start with stating that I had an even more detailed post written up that was erased when I clicked "Post" and was auto redirected back to the login page and could not recover my post after logging in (again). I am frustrated by this occurrence; anyway:

I work on site with one other twin machine and an older 2010 MP. There is also an offsite 2010 MP that is a near twin (different GPU) of the onsite 2010 MP. All machines are on OSX 10.9.5 and MC 7.0.6.3.
In recent months (since around the time we started using dedicated audio mixers for recording and updated to 7.0.6.x from 7.0.4 which occurred at around the same time) we have started seeing this error with seemingly increasing regularity. It occurs sometimes during playback, frequently during export attempts and occasionally with consolidation attempts (all consolidation is done on one of the 2013 MPs.)
The media in question is EX3(video) and SD664(audio--sometimes others as well) transcoded offsite to H.264 800kbps MXF proxy media on Windows MC 8.x machines and then synced to me via the internet.
The media is stored on 4TB FW800 G-Drives and is sometimes also read off of 1TB USB3 MyPassport drives. Usually one sequence will be reading off of at least two of the G-Drives.
My scouring of the forums and the internet at large has lead me to believe that the error is related to the media, and perhaps specifically the .mdbs. I have had some occasional success making avid reindex all of the media prior to an export attempt, but this is very tedious in the case of larger projects (is there a function to have MC reindex all available media?).
We have also band-aided the issue by using the "Change Sample Rate" function on problematic sequences, but they seem to revert to kicking back the same error even after only minor changes. We have now on a few occasions had to revert to exporting longer sequences in chunks which we then stitch together with QuickTime or similar after the fact.
The error seems most prevalent (and VERY persistent) on sequences that were built using the on site 2010 MP and will continue to persist even if the media and sequence are transferred to one of the newer machines. It also occurs occasionally on the 2013 machines. I have never had this error reported by the off-site 2010 MP (which is weird as they use the same media, on the same type of drives, processed in the same way).
1. Are there any obvious culprits based on what I have explained that make for easy fixes/would be fixed with work flow changes?
2. Has anyone else seen an increase in the occurrence of this error?
3. Is this a documented error that was remedied in a more recent MC release? I'm hoping to make use of a lull around the start of 2017 to get us updated to MC 8.x (anyone have a source for a Mac Pro Avid MC compatibility chart? I worry for the older 2010 machines).
Any insight or speculation is welcome, I'm tired of dealing with this thing.
